چکیده
In case of locally advanced soft tissue sarcomas the extremities (STS), large size which does not allow resection with preservation limb, isolated regional perfusion (IRP) limb is considered as one effective treatment options. Aim. To evaluate long-term outcomes IRP melphalan under conditions hyperthermia for STS extremities. Materials and methods. We conducted a prospective cohort clinical study that included 42 patients All underwent melphalan, in – combination tumor necrosis factor alpha. Additional lymph node dissection was performed 8 cases (19%). The 5-year overall survival estimated by Kaplan-Meier method. Results. median age 50 years (from 24 to 80 years), women predominated 32 (76%). upper diagnosed 14 (33%), lower 28 (67%). G3 grade verified 34 (81%) patients, rest G2. Most had recurrent cases. received previous treatment: surgery chemotherapy / or radiation therapy 36 (86%), alone 2 (5%), 4 (9%) patients. Within 5 years, recurrence developed 20 (48%) amputation 10 (24%) distant metastases were (10%) mean time relapse after IPC 9.3 ± 3.5 months. Overall at 1, 2, 3, 4, 98%, 88%, 86%, 74%, 67%, respectively. Conclusion. an method extremities: rate 67% low amputations metastasis.

Ifosfamide with regional hyperthermia in soft-tissue sarcomas.
For high-risk soft tissue sarcomas (HR-STS) of adults, new treatment strategies are needed to improve outcome with regard to local control and overall survival. Therefore, systemic chemotherapy has been integrated either after (adjuvant) or before (neoadjuvant) optimal local treatment by surgery and radiotherapy in HR-STS. The combination with regional hyperthermia as a new treatment strategy s...
متن کاملHyperthermic isolated limb perfusion for extremity sarcomas.
BACKGROUND The treatment options available for extremity sarcomas are amputation or limb-sparing surgery with radiation, which may incur significant morbidity and body disfigurement. Hyperthermic isolated limb perfusion (HILP) may be an attractive option in extremity sarcomas for unresectable lesions to preserve limb function and maintain quality of life. METHODS We report the outcomes of 5 p...
متن کاملThe pathologic response of resected synovial sarcomas to hyperthermic isolated limb perfusion with melphalan and TNF-α: a comparison with the whole group of resected soft tissue sarcomas
BACKGROUND Hyperthermic isolated limb perfusion with tumor necrosis factor-α and melphalan (TM-HILP) has been successfully used to treat limb soft tissue sarcomas (STSs) with high response rates. The data on the effectiveness of HILP-TM for the treatment of STSs are mainly based on various STS types. The aim of this study was to investigate the responses of synovial sarcomas (SS) to TM-HILP. ...
متن کاملOutcomes of isolated limb perfusion in the treatment of extremity soft tissue sarcoma: a systematic review.
BACKGROUND Isolated limb perfusion (ILP) may provide a limb salvage option for locally advanced soft tissue sarcoma (STS) not amenable to local resection. METHODS A systematic review was performed for studies reporting outcome of ILP for locally advanced STS performed after 1980 in patients aged ≥ 12 years old. The main endpoints were tumour response and limb salvage rates. Complication and r...
